Dear Parent or Guardian,

 

Your son/daughter is eligible to participate in a church sponsored
activity.  This activity will take place under the guidance and
supervision of employees and/or volunteers from
Saint Elizabeth Ann Seton Parish. 

A brief description of the activity follows:

 

Name of  Event: Snowtubing

 

Destination: Ski Roundtop, Lewisberry, PA

  

Designated Supervisor(s) of Activity: Mary Brenner and Youth Group Staff

  

Date and Time of Departure: Jan. 20, 2008, 5:30 p.m.
from the St. Elizabeth Ann Seton parking lot.

  

Anticipated Time of Return:  8:00 p.m.

  

Method of Transportation: private vehicles

  

Cost: $16.

  

Additional Information: Dress appropriately for snow sports

     (hats, gloves, water-resistant clothing).

                                         Your child may want to bring extra cash for snacks.


Ski Roundtop release form is also required (available at church).
